1. Principal Illness Navigation (PIN) Program Support

Overview

HealthArc is introducing support for the Principal Illness Navigation (PIN) program using the existing Care Plan framework.

PIN allows care teams to document navigation and support activities, track time spent with patients, and manage PIN-related documentation within the same familiar workflow used for other care management programs.

The PIN program can also be used alongside other HealthArc programs, including RPM, RTM, CCM, PCM, APCM, and BHI, when clinically appropriate.

What is PIN?

Principal Illness Navigation (PIN) is designed to support patients who require additional assistance navigating care related to a serious or complex illness.

Through PIN, care teams can help patients with activities such as:

  • Navigating their healthcare journey

  • Coordinating care and services

  • Connecting patients with appropriate resources

  • Supporting follow-up activities

  • Addressing barriers to care

  • Helping patients understand and access available services

  • Providing ongoing support throughout the month

HealthArc provides a centralized workflow to document these activities and track the time spent providing PIN services.

How to Enable PIN in HealthAarc

PIN can be enabled for both new and existing patients.

New Patient

Onboarding Form

Go to Onboarding Form → Health Conditions → Enable PIN. Add the applicable chronic/serious conditions that will be monitored as part of the patient's PIN care.

Existing Patient

Edit Patient

Go to Edit Patient → Health Conditions → Select PIN. Add the applicable chronic/serious conditions that the care team will monitor and support through PIN.

Once PIN is enabled, the patient can be managed through the PIN workflow within the Care Plan framework.

PIN Program Workflow

Step 1: Enable PIN

Enable the PIN pr

ogram for the patient through the onboarding or Edit Patient workflow.

Step 2: Add Applicable Conditions

Add the chronic or serious conditions that the care team will monitor and provide navigation support for.

This helps establish the clinical context for the patient's PIN activities.

Step 3: Document PIN Activities

Care teams can document activities performed as part of the patient's PIN program, including navigation, coordination, follow-up, and other applicable patient support activities.

Step 4: Track Time

HealthArc tracks staff time spent on PIN activities.

Time can be accumulated throughout the calendar month and associated with the appropriate patient and PIN program.

Step 5: Review Monthly Time

The accumulated time can be reviewed as part of the patient's PIN workflow to determine whether the applicable monthly billing requirements have been met.

Step 6: Billing Support

HealthArc supports the applicable PIN billing codes based on the documented time.

The system distinguishes between PIN and PIN-PS activities to support appropriate tracking and reporting.

PIN with Other HealthArc Programs

PIN does not need to replace other care management programs.

A patient can be enrolled in PIN alongside other applicable HealthArc programs, such as:

  • RPM

  • RTM

  • CCM

  • PCM

  • APCM

  • BHI

Each program's activities and time are tracked separately to maintain clear program-level documentation and reporting.

Reporting & Documentation

The necessary PIN-related information will be included in the applicable monthly billing reports to support billing review and reconciliation.

The report will distinguish between PIN (G0023/G0024) and PIN-PS (G0140/G0146) where applicable.

2. Add Secondary Insurance Details to Patient Enrollment Bulk Upload

Description:
Enhance the Patient Enrollment → Bulk Upload template to allow users to capture secondary insurance information during bulk patient enrollment.

New Columns:

  • Secondary Insurance Name

  • Secondary Plan Policy Number

Requirements:

  • Both fields should be optional/non-mandatory.

  • Patients can be uploaded successfully without providing secondary insurance information.

  • When provided, the information should automatically populate the corresponding fields in the patient's profile.

Business Benefit:
The client frequently uses the bulk enrollment workflow and would like to capture secondary insurance information during the initial upload. This will eliminate the need to manually edit individual patient profiles after enrollment and reduce administrative effort.

3. Controlled Access for RTM Manual Readings

HealthArc now supports manual RTM reading entry for selected practices, providing an alternative for patients who may not be able to use mobile or SMS-based monitoring.

What's New?

  • Manual RTM reading entry can be enabled for selected practices through a backend setting.

  • Once enabled, authorized users can manually enter RTM readings on behalf of patients.

  • Manually entered readings are clearly identified and audit tracked.

  • Existing RTM SMS workflows remain unchanged.

  • Supports elderly or less tech-enabled patient populations.

  • Provides greater flexibility for RTM data collection.

  • Maintains appropriate access control and auditability.

3. Secondary Insurance in Patient Enrollment Bulk Upload

Q: What has been added to the bulk upload template?
Two new optional columns have been added:

  • Secondary Insurance Name

  • Secondary Plan Policy Number

Q: Are these fields mandatory?
No. Both fields are optional. Patients can still be uploaded without providing secondary insurance information.

Q: What happens when secondary insurance information is provided?
The information will be captured during the bulk upload and added to the patient's corresponding insurance details.

Q: Why was this enhancement introduced?
It allows practices that frequently use bulk enrollment to capture secondary insurance information during the initial upload, eliminating the need to manually update individual patient profiles afterward.

Q: Will this change affect existing bulk uploads?
No. Existing bulk upload workflows will continue to work, and the new fields can be left blank when secondary insurance information is not available.
