  1. Dexcom Web-Based Integration & Authorization

    HealthArc now supports a web-based Dexcom connection, allowing practices to connect a patient's Dexcom device directly through the HealthArc portal without requiring the HealthArc mobile app.

    How It Works

    1. Add the Patient
    Add the patient to HealthArc as usual. Under RPM Setup, select Glucose as the monitoring parameter and select CGM (Dexcom).

    2. Open Device Setup
    Open the patient's chart and select Edit Patient. Navigate to the Device Setup section, where you will see the Connect button next to the Dexcom device.

    3. Connect Dexcom
    Click Connect and select Connect with Dexcom from the window that appears.

    4. Log In & Authorize
    Enter the patient's Dexcom username and password and follow the prompts to authorize the connection. Once successful, a confirmation message will appear.

    5. Confirm & Start Monitoring
    Return to the HealthArc portal. The Connect button will appear disabled, confirming that the connection is active. Once connected, the patient's Dexcom glucose readings will automatically flow into HealthArc.

    Quick Workflow

    Add Patient → Select Glucose + CGM (Dexcom) → Edit Patient → Connect → Connect with Dexcom → Patient Login & Authorization → Dexcom Connected → Glucose Data Flows to HealthArc

  2. Sort Care Plan Patients by Last Care Plan Update

    We have added sorting functionality for the Last Care Plan Update Date across applicable Care Plan program dashboards. This makes it easier for care teams to identify patients whose Care Plans were updated recently or those who may need an update.

    How It Works

    • Open the applicable Care Plan dashboard
      Navigate to the program dashboard, such as CCM, PCM, APCM, BHI, or MTM.

    • Locate the Sort by Column next to Filter

    • Choose the required order

      • Earliest to Latest – Patients with the oldest Care Plan updates appear first.

      • Latest to Earliest – Patients with the most recent updates appear first.

    • Prioritize your workflow
      Care teams can use the sorting to quickly identify patients who may require a Care Plan update or review.

    Example

    If the patient list includes Care Plan update dates of:

    • January 10

    • March 15

    • June 20

    • August 31

    Selecting Earliest to Latest will show January 10 first, while Latest to Earliest will show August 31 first.

    The sorting functionality is available across applicable programs dashboard supporting Care Plans

    Q: What is the new sorting functionality?
    Users can now sort the Last Care Plan Update Date column to organize patients based on when their Care Plan was most recently updated.

    Q: What sorting options are available?
    You can sort the list by:

    • Earliest to Latest

    • Latest to Earliest

    Q: Where can I use this functionality?
    It is available across applicable Care Plan dashboards, including CCM, PCM, APCM, BHI, and MTM.

    No. This enhancement only adds sorting functionality to the existing Last Care Plan Update Date field.

    Q: Will existing dashboard functionality be affected?
    No. Existing dashboard workflows and functionality remain unchanged.
