1. Multi-Select Filter Support Across the Portal:


Description: The portal’s filtering capabilities have been upgraded to support multi-select options across key modules, significantly improving user efficiency and experience. Users can now select multiple filter values simultaneously instead of applying filters one by one.

Scope of Enhancement:
✅ Dashboard
✅ Patient Listing

Key Features:

Multi-Select Functionality

· Users can choose more than one value within any filter.

· Filters will intelligently apply AND/OR logic depending on context.

Improved User Experience

· Checkboxes are added for easy selection.

· “Select All” option included where applicable.

· Clear Filters option resets selections without reverting to Page 1 — the current page context is maintained.

2. Add “Apply to All Patients” Button in RPM Settings:

Description: A new “Apply to All Patients” button has been added in the RPM Settings, allowing Admins to quickly apply the same RPM monitoring parameters across all existing patients. This feature enhances efficiency and promotes consistent configuration.

Scope of Implementation:


RPM Settings → Apply uniform parameters to all patients


Key Features:


Apply to All Patients Button:

  • Adds a centralized button within RPM Settings

  • Triggers a confirmation pop-up before proceeding.

  • Once confirmed, the system updates RPM parameters for all existing patients.

3. Request for Ability to View Past Care Plans:

Description: Care teams can now view all past care plans, even when a patient switches programs. This ensures continuity of care, preserves clinical history, and supports better-informed decision-making.

Key Features:

All previously created care plans remain accessible, regardless of program transitions.

Users can easily toggle between current and past care plans within the patient profile.

4. Ability to pin notes in a patient’s chart:

Description: Care team members can now pin critical notes in a patient’s chart within the Healtharc Portal. This feature ensures that high-priority information—such as allergies, care preferences, or urgent instructions in the Notes—remains easily visible at the top of the Notes section.

Key Features:

✅ Pin/Unpin Functionality:

· Each note includes a pin icon to allow quick pinning/unpinning.

· Pinned note is moved to the top of the list for high visibility.

· When the note is unpinned, we get a confirmation pop-up, as below:


· Unpinned notes return to their original chronological order.

✅ Pinned Notes Display:

· Pinned notes appear under a clearly labelled 📌 "Pin icon.”

· Only one note can be pinned at a time, and when the next note is pinned, it replaces the previous one by default.

5. Care Plan Tab Update: Section Rename & Split:

Description: The "Psychosocial and Neuropsychological Testing" section in the Care Plan tab will be replaced and split into two distinct sections for improved clarity and clinical alignment.

Updated Sections & Prompts:

  1. Cognitive Status & Mood:

    • Question: Has the patient been assessed for cognitive status (alert and oriented) and mood?

  1. Social Determinants of Health (SDOH):

    • Question: Has the patient been assessed for Social Determinants of Health (SDOH), and do they need any assistance in this area?
