Introduction of APCM (Advanced Primary Care Management)

What’s New?

We are excited to introduce Advanced Primary Care Management (APCM) - a new Medicare-supported care coordination module within HealthArc. APCM enables providers to proactively manage patients with one or more chronic conditions, under a structured, reimbursable care delivery model.

This release mirrors the proven CCM (Chronic Care Management) workflow, while adding enhanced capabilities to broaden support for chronic care under the APCM umbrella

Key Highlights:

1. New APCM Workflow

a. Fully replicates the existing CCM care plan structure.

b. Maintains identical user roles, permissions, and task management logic.

c. Enables effortless transition for existing CCM users.

2. Billing Code Support

a. Integrated with Medicare billing codes for APCM:

i. G0556 - For patients with one chronic condition.

ii. G0557 - For patients with two or more chronic conditions.

iii. G0558 - For Qualified Medicare Beneficiaries (QMB) with two or more conditions.

b. A new QMB checkbox has been added to the Insurance section on the Add/Edit Patient screen. Enabling this allows the use of G0558 billing code.

3. Program Interoperability

APCM works seamlessly alongside other care modules:

· RPM (Remote Patient Monitoring)

· MTM (Medication Therapy Management)

· TCM (Transitional Care Management)

· RTM (Remote Therapeutic Monitoring)

· Full billing compliance with APCM-specific CPT codes.
